Hello - who said that it was from stress and pain and were they meaning to give you a diagnosis  or was it a suggestion?  Stress and pain can result in diarrhoea but in general, to come to this conclusion, one would have to rule out bacterial causes, Viral causes, tumours, internal medicical causes (liver disease, pancreatitis, worms, etc) and this would take a lot of investigation. Good questions for your vet include:  what is causing the diarrhoea?  Is it bad enough that it needs to be treated in order to prevent fluid loss (eg fluids)?  Would it be a good idea to try to investigate the diarrhoea to any extent before performing major orthopaedic surgery - as it would be heartbreaking to put a kitten through the surgery and then find out that the diarrhoea represents a life changing complication?  It is possible that the vet has thought all this through in the scenario and that talking the case through with you could help to put your mind at rest.

It follows on logically that if a cat is in pain bad enough to cause diarrhoea, they might be put on some kind of pain relief in order to help.

Having looked at the radiograph. I can see why your vet would have concluded that your cat may well be in pain.  Did they make a good response to pain relief?
