Hello - I wonder if this is a change to both sides of the body symetrically, accompanied by some hair loss, in which case I would wonder about underlying hormonal diseases such as thyroid disease, Cushings or ovarian tumours. Particularly if your dog is unspeyed but in any case, hormonal diseases are well worth asking your vet about.
Other causes of small lumps may include demodex mites (often associate with a drop in immunity - sometimes as a result of endocrine or other disease), bacterial or yeast infections. Fly attacks, autoimmune diseases or allergic reactions may be possibilities, as may cancer.
This is strange rash is well worth a chat to your vet about.
