Topical (fipronil) plus flea collar (deltamethrin) for dog bathed frequently?

We have 3 dogs and are trying to stop an flea infestation. All dogs are currently getting topical fipronil treatments. The oldest dog is a ~12 pound terrier-chihuahua mix, 16 years old, who gets baths (miconazole plus chlorhexidine) every 1-3 days for chronic dermal yeast problems . The fipronil does not seem to work well for him, perhaps because of the frequent baths, and it may be causing him to "mop up" the fleas from the other dogs and prolong the infestation. Would it be OK to try a  deltamethrin collar on him in addition to the fipronil topical treatment? He is old and not strong, with failing vision and hearing, but he eats well. I'm trying to not overtreat him to avoid any side effects, but he needs a more effective treatment. Thanks. tcpasley

Hello - this question raises more question than answers at this stage, I'm afraid.  You don't mention the cause of the chronic dermal yeast problems, but usually there is one; it would be surprising for healthy skin on a healthy pet to suffer persistent yeast infections to this extent. I therefore wonder whether other treatment is needed for some other disease process.  Regarding bathing, I know that many spot-on preparations suggest that patients are not bathed for the first 24-48 hours while the product takes hold (it has to be absorbed across the skin), but once absorbed it ought to remain effective.  There are other reasons for failure of flea control; resistance, while being widely talked about, is not currently thought to be the most likely. According to Ian Wright, writing in the Veterinary Times in 2022, pitfalls to look out for include:  insufficient frequency of administration; inefficient application of product (eg following weight changes, or with respect to the application method); cats visiting the household that are not being treated (most fleas of both dog and cats turn out to be so-called 'cat fleas');  incomplete removal of fleas from the environment (so they lie dormant and then leap back on again as soon as it seems clear); even other kinds of flea, because cat fleas (the dominant type) are not the only ones.  Finally, you do not mention how you know that the flea treatment is not working;  obviously if there are fleas on your dog, then it is obvious, but if you are seeing scratching then it can be attributed to a long list of other causes eg allergy.   With all this in mind, it seems a good idea to return to your vets for a review of flea control within the household and in particular to let them assess your golden oldie.  It could be that treating on the dot every month ends up being the most effective strategy.   The prescribing rules for flea treatment for dogs are about to change and therefore, if they do not need to see your dog again in a hurry, they may ask to put up several prescriptions in advance, which is different from the current system.  I hope that something there helps.
