Hello - its hard to narrow down without a full history and perhaps examining your dog, knowing his breed, age, home situation and so on. This sort of information should be collected systematically by your vet, together with a thorough examination.  However, I do wonder whether he might be in pain; whether he might be experiencing vision or neurological problems, or even systemic eg liver changes that lead to secondary effects.  It's certainly worth a veterinary appointment to investigate, althought there might turn out to be a few stages to pinpointing a diagnosis or figuring out precisely what else is behind these new, currently quite general signs.  Wishing all the best to you and him at this very anxious-sounding time.
