Hello again - I think at this point, your vets are in a position to know better than me. They have, after all, seen the scan and the bloods. From here for example, I am in no position to rule out pancreatitis as a main cause, having seen dogs with pancreatitis with fever. Is there actual evidence of a bacterial infection? Because your vet can see more than I can, suggestions are best coming from them now. Good questions might be: could a biopsy be benefitical? Have pain killers been tried? Might cancer be a on the list? What differentials could explain everything you can see (more than one route cause, as they say, is always a strong option) including the blood and the eye. As a vet, if I am ever stuck with a medical case, I will often call a pathologist. Pathologists get to know about the 'interesting cases' from a large nunber of surgeries and are often good people to say 'those signs / dog that age / presenting like this / here's what's most likely / here's the best test.'
I hope that you get to the bottom of this interesting case.
