Hello and thank you for this interesting query. Its always worth saying that although a lot of inflammatory reasons are red or painful, not all of them seem that way - and cancer frequently isn't. Many dogs, however, do naturally have a surprisingly big epididymis. It can be useful to stand the dog up and check the testicles from different angles, to see whether or not there is a matching pair. If the two don't match, we would firmly reccommend seeing your vet.
