Hello! One thing I do know about this is that it needs a vet appointment, because scratching will damage the skin and tissue on and around it, leading to pain and infection, leading to more scratching..... And indeed, the longer they are left, the secondary effects of such lesions become worse and harder to heal.
However, you asked what it is. Possibilities include: an extra nipple, got infected; a tumour (got infected; they don't usually itch); a histiocytoma (strawberry lump); possibly a hernia (doesn't look like one from this angle?); an exaggerated reaction eg to a fly bite or foreign body..... It is likely that your vet may not know for definite on the first presentation, but this is ok - they will help to formulate a plan as to what to do next.
