What are these open sores near my dogs lips?

Oooh my goodness, your dogs' lips look sore! Whatever is causing it, pain relief might be necessary pretty urgently! I also wonder about uraemia (eg resulting from kidney disease) which can be life-threatening, and viral infection; you should call your vet for triage (to decide how urgently this case needs to be seen).
