Help, advice, considerations for diagnosis; Female Cat, 3yr, spayed, full VAXed

First, I want to make it clear that I fully understand online advice is NOT a diagnosis and will not consider it as such. I have been seeking veterinary help in person at the local clinic but has not been producing any diagnosis at all despite thousands of dollars spent in checkup. Initial signs: Anorexia, fever, lethargy, dehydration Initial treatment/checks: In house blood panel (everything within normal ranges, no elevated WBC), 2 rounds of x-ray (found only her poop moving, organs seem okay), Temp. 104 F, Given IV fluids, anti-nausea injection, take home Clav drops and Rebound electrolytes. After administering clav + rebound, no improvement in appetite or behavior 2nd visit: Full panel blood work, sent to IDEXX for Fever of Unknown Origin Real PCR, negative for everything. Tried Mirataz for appetite stimulation, no improvement.   Talked with vet again today (now ~1 week since initial visit, 1+ week since pet stopped eating), they recommend to do another blood work. I've been doing online research, and pancreatitis seems possible? But the doctor doesn't seem to be concerned about it? Current symptoms: Dehydration, severe weight loss, lethargy, anorexia, possible fever, glassy eyes + third inner eyelid not fully retracted, no vomiting or diarrhea. Only eats churu sticks occasionally. Been following Clav drops + rebound instructions to the T, plus forced hydration via syringe. Hello and thankyou for this fascinating question and understanding how limited our information is as online vets, unable to observe the patient or see the history.  Sounds to me that your cat is pyrexic (fever of 104), anorexic and has lost weight.  I am not sure either why pancreatitis has been ruled out on the basis of what you've told me; nor do I understand from here at this stage the basis for giving antibiotics.  Not all pancreatitis cases vomit or have diarrhoea; not all fevers are caused by infection (pain can do it) and a week without eating sounds to be a concern.  Have you asked the vet about this, or about why they want to do more blood work and what for (because of course, they know more about your cat that me)?  Is there a diagnosis they think likely at this stage?  Also concerning, is a lack of trust between you so I do wonder whether a second opinion from a medical expert might be useful?  - if your vet is on the right track, that could prove very reassuring and if not, early intervention / even feeding tubes may be suggested.  Wishing you all the very best - please will you let me know how you get on?
