Hello - I'm going to be honest: I have never seen anything looking exactly like this (I am in the UK, to be clear) and I suspect that your vet, while having more context, may also need to do tests to know which of several conditions with completely different causes, they are looking at. I can see a deep ulceration to the side of the nose near the eye and also ulcerations to the junction where the inside of the eye meets the outer skin. It may be that the lesion on the nose is secondary to the eyelid lesion (e.g. if the eye was painful or itched), or vice versa, or it may be that they both occured together. Demodex mites can, occasionally, cause unexpectedly sore-looking lesions to the eyelids, but demodex themselves rarely itch. Sarcoptes mite species may itch, however. There are potentially infectious causes e.g. bacterial causes, such as streptococcus. Apparently there is a species of weevil, cuterebra, that can cause similar lesions, although I found this from a picture in Veterinary Practise and have never seen it. There may be autoimmune conditions that could be responsible. Autoimmune conditions such as lupus, or even pemphigus, tend not to itch at first but again can cause sore-looking lesions. Finally, cancer might be a possibility, for example squamous cell carcinomas or even eyelid melanomas.
