Stray cat very skinny and limping

I've been feeding this stray cat for a few months, but suddenly he disappeared for a week. When he returned, he was very skinny, limping, and his injured leg was also very dirty. I've also noticed that he has a very hard time chewing now. Should I take the little guy to a vet? See video below: https://streamable.com/rhp8os   Any answer would be appreciated.
