Hello and thank you for this interesting question. It is not always necessary to shave a dogs' testicles, especially if it gives rise to soreness. Other causes of scrotal inflammation connected to rubbing may include fleas, mites or nicks in that area, or pain eg in the event of lumps or acute injury such as testicular torsion; be especially aware of the latter as it can be quickly fatal. Furthermore, prostate disease and sometimes bladder infection can present in this way. Assuming the cause to be as you described, even this must be extremely sore and soreness leads to further licking, creating a vicious cycle; pain killers might be necessary (one appropriate to that patient; please note that some common human pain killers can be fatal in other species). Therefore, in balance, we feel that you do need to see a vet, perhaps relatively soon as this condition must involve some degree of distress. Antibiotics, however, may not be necessary.
