Persistent urinary incontinence after accident

Small dog hit by car 4 months ago. Walking recovered almost completely (he basically couldn’t walk or move for 2 weeks), but urinary control did not. At first he had urinary retention. Now the bladder usually does not feel full, but he leaks urine, mainly when excited or while walking. Bethanechol made leaking much worse. One anti-inflammatory + antibiotic course helped a lot for 1–2 days, then symptoms returned. Another antibiotic/anti-inflammatory course did nothing. Urinary food did not fix it. Stool is normal, but he sometimes seems to start pooping before fully choosing where to go. Does this sound like neurogenic incontinence/storage failure rather than overflow? Would PPA be worth asking my vet about? What tests should be done next?
